What are your arrival/departure dates and times in Boulder?

A) Will you be driving your personal vehicle or rental car to and from class while in Boulder? Yes No

If so, would you be willing to be a driver of one of the COMET minivans during the course? Yes No

If so, you will need to sign and submit a signed copy of the Van Usage Policy, and provide the COMET Program with a legible copy of your driver's license.
