Here are some instructions on how to fill out a questionnaire fill-in form.
It is important to note that you must press the Submit Comments button at the bottom of the form to save your results when you are finished. If you do not do this, your results will be lost when you exit the questionnaire.
For your convenience, you may submit only a partially completed form. You can then exit the questionnaire and return later to complete it and submit the rest of the form. The questionnaire form will regenerate the most recently submitted comments. Thus, if you change your mind about something you can edit previously submitted comments.
Fill in your four digit id in the appropriate box.
If there are radio buttons, check the button to mark your choice.
To fill in text blocks, locate the mouse pointer in the text block area and start typing. There are horizontal and verical scroll bars to view a longer comment block. Hit the Return key to carriage return/linefeed to the next line.
